Dr. Fernando Munguia

Late Founder, Prosthodontist, Master in Oral Medicine and Orofacial Pain

22 years ago, with a passion for helping people attain their dream smile, the late Dr. Munguia founded “Smile Prosthodontics,” now known as California Smile Dentistry. Dr. Munguia’s love for his profession led him to teach at Loma Linda University and across the globe as an international speaker, as well as training & mentoring numerous graduate students and dentists in the area. Dr. Munguia had several specialties under his belt from Loma Linda University (LLU) and the University of Southern California (USC), including Orofacial Pain & Oral Medicine. He was very engaged in the dental community, and committed to giving back to it. He was a member of the American Board of Prosthodontics, and a Fellow of the American College of Prosthodontics. 

Dr. Munguia passed away in November 2023, but his legacy lives on in the practice’s commitment to delivering patients their dream smiles.

Dr. Jackqueline Martinez

Managing Dentist

Dr. Martinez received her California DDS degree in 1994, and in 2014 she received her Master’s degree from the University of Munster, Germany. She worked alongside Dr. Munguia (Founder of California Smile Dentistry) for over 20 years and assisted him in countless implant surgeries and restorations. She is very familiar with the fabrication of ” All on X” fixed prosthesis, as well as all cosmetic procedures. As a result of working alongside Dr. Munguia for over 20 years, Dr. Martinez has a lot of experience with implants, implant-related restorations, and cosmetic procedures. 

Dr. Martinez spends her free time with her 3 children, and she enjoys meeting people and learning new cultures and languages. She also volunteers in global dental mission trips.

Dr. Mario Flores

Periodontist & Implantologist

Dr. Mario Flores practiced as a General Dentist for 10 years before obtaining his Periodontics Specialty, and has since been in private practice for almost 40 years. He and Dr. Munguia had a personal and professional relationship for many years. Nowadays, Dr. Flores is a Full-time Faculty at Loma Linda University. As a resident of Rancho Cucamonga along with his wife and son, he is an active member of the community and he enjoys biking and teaching.

Dr. Fahad Warriach

Implantologist

After completing dental school at the University of Lahore in 2012, Dr. Fahad Warriach received his DDS degree and later his specialty in implant dentistry. Dr. Warriach also holds his Sedation Certificate. Dr. Warriach brings 12 years of dental experience and a happy heart. He is very devoted to his patients and is honored to join our practice. When he’s not working, he enjoys spending time with family & friends, and watching a game or two of cricket, his favorite sport.

Dr. Yaczaira Perez

General Dentist

A native Puerto Rican, Dr. Perez obtained her Bachelors degree in Biology from Pacific Union College in 1998. She received her dental degree from Loma Linda University in 2002. Outside of school and the office, Dr. Perez has done dentistry missionary work in Thailand, Mexico, Bangladesh, Honduras, and Guatemala, among others. She is happily married and has three children. When off work Dr. Perez enjoys camping, traveling, playing the piano and spending time with family.
